Emergency care for pulmonary hemorrhage![]() Author: ambulance doctor Deryushev A.N. Pulmonary bleeding is the release from the respiratory tract during a cough of clean blood or blood in the form of abundant impurities to the sputum. Pulmonary bleeding is a serious and dangerous complication of various inflammatory, specific or nonspecific respiratory diseases, as well as malignant tumors and lung damage. Bleeding begins with the disintegration of lung tissue, when the vessels do not have time to be thrombosed. In addition, foreign bodies in the trachea, bronchi and lungs, which injure the vessel or cause its damage in the form of a pressure sore, can cause such bleeding. By the amount of blood allocated, such bleeding is divided into small (not more than 100 ml of blood), moderate (100 to 500 ml), and profuse, when more than 500 ml of blood is released. Pulmonary bleeding usually begins suddenly, or after hemoptysis. The secretion of blood is usually pink and red, frothy, if before that the blood was delayed for some reason in the lungs, then its color will be dark brown, sometimes with clots, but there may also be scarlet foamy blood. Blood discharged from the esophagus with gastric or esophageal bleeding, coughing is not accompanied. Insignificant blood loss an adult can tolerate easily, but in children and the elderly, even minor bleeding can disrupt the vital activity of the whole organism. Treatment of pulmonary hemorrhage
The treatment of pulmonary hemorrhage consists in possibly stopping bleeding earlier and compensating for significant blood loss. Among the hemostatic drugs, dicinone, or its domestic counterpart, etamzilate, is most often used today. With significant blood loss already at the prehospital stage, it is necessary to perform venous access and begin intravenous drip infusions. For this, the use of aminocaproic acid is possible. The use of calcium chloride, which has the ability to stabilize the cell wall, including the cells of the vessel wall, is thereby justified, thereby preventing diapedesis (impregnation) of blood through the vascular wall. It is also possible to use the styptic drug Vicasol, but it must be taken into account that its effect develops slowly, within six hours after administration. To restore the volume of circulating blood in the prehospital stage is possible by any available solutions, starting with the usual 0.9% saline solution. However, it is more effective to use modern drugs that are able to fight with a threat or even with a shock that has started on the background of acute blood loss - it is gelofusin, venofundin. Blood pressure control is mandatory, as high blood pressure can increase bleeding, and then ganglion blocking drugs (pentamine) are advisable, and low blood pressure can indicate a hemorrhagic shock that starts, which is a very serious complication of pulmonary hemorrhage. Clarification of the cause of pulmonary hemorrhage makes it possible to determine the tactics of treatment, since it is necessary to act if it is possible on the disease itself, which contributed to the appearance of bleeding. After assisting in the prehospital stage, the patient with pulmonary hemorrhage should be taken to the nearest medical institution.

I’m in the ER right now with multiple bilateral pulmonary emboli, from a DVT in left leg behind knee. I got it by sitting at a desk working for 10 hrs/day. And a cross-country flight.
Linda Wilkinson:
Just got out of the hospital from pulmonary embolism, still have a long way to go to feel better. Still have pain and SOB if I move to much. The doctor who sent me to the ER said you could die from this, I believe him now.

JIMJAMSC:
FYI for anyone concerned about DVT and clots. I had NO warning or signs but do have high blood pressure and high cholesterol. Woke up and my left leg was swollen, foot had no definition, calf hard as granite and leg was warm. DON'T WAIT DAYS like I did. It's not a strain. I had a massive clot and a CT revealed TWO lung embolisms. Using a catheter/vac they removed 3 and 1/2 ft of thrombus "clot" and I will be on blood thinners forever. Also wear compression stockings. Athletes do and they really work both in preventing DVT and preventing a 2nd event.
