Angina pectoris and myocardial infarction refer to such a common concept as ischemic heart disease. Coronary heart disease (CHD) is characterized by a mismatch between the intake of blood rich in oxygen to the cells of the myocardium and the oxygen demand for these cells. This lack of oxygen generates oxygen starvation (hypoxia), and then ischemia of cells and tissues. With a heart attack, this process ends with necrosis.

Thus, angina pectoris is a clinical syndrome, which in the patient manifests a characteristic pain, which is associated with transient short-term ischemia of the myocardium. The pains behind the sternum with angina are compressive, squeezing, appear more often with physical overload or emotional stress, and after the termination of the load, or after taking nitrates (nitroglycerin). With the progression of the process, pain can also appear in rest - rest angina. Pain with angina pectoris can have a fairly characteristic sign - irradiation, that is, in simple words, such pain "gives" to the left shoulder, left arm, left shoulder blade, under the left collarbone, to the left side of the neck. Other symptoms of angina pectoris may be shortness of breath, worried about fatigue during exercise. The duration of an attack of angina may be from 1-5 to 10 minutes.

Before the arrival of an ambulance team, or before the patient's first contact with a medical worker, it is necessary:

1. Lay the patient with an elevated head end, provide warmth and comfort, free breathing, calm the person.

2. Give the patient a drug from the group of nitrates, taking into account what kind of medication the patient used earlier - it can be a tablet of nitroglycerin, under the tongue, or a drug in the form of a spray - for example, Izoket. If necessary, repeat after 5 minutes. Usually, a bout of pain after this stops.

3. If the painful attack lasts more than 15 minutes and the purpose of nitrates is not stopped is an occasion for contacting the emergency medical service, or for any medical worker.

4. With continued seizure, it is advisable to give the patient chewing half a tablet of aspirin - 250 mg (acetylsalicylic acid)

The general condition of the patient is necessarily assessed - blood pressure and heart rate are measured, if possible, an ECG can be recorded.

Absence of nitrate effect, and prolonged pain attack indicate the presence of unstable angina in the patient, or the onset of the formation of acute myocardial infarction.

Acute myocardial infarction is an urgent clinical condition, also caused by acute myocardial hypoxia, however, the necrosis (necrosis) of the heart muscle begins to form. The necrosis zone corresponds to the zone of blood supply of the affected vessel and is determined after 4-6 hours from the onset of ischemia.

Pain in infarction, as a rule, have a greater intensity than with angina and more often have a burning character, the duration of such an attack exceeds 15 minutes (and this is also a diagnostic sign of a heart attack). Nitrate intake does not relieve pain. Noticeable pallor of the skin, often a cold sticky sweat, general weakness, a sense of lack of air. This is a typical picture of an infarct, however, there may be other variants - there are painless forms, with irradiation of abdominal pain (abdominal form), arrhythmic form, etc. First aid tips are as follows:

1. Lay the patient with a slightly elevated head end

2. Ensure absolute bed rest, since any physical load can provoke such formidable complications of heart attack as cardiogenic shock or pulmonary edema.

3. Ensure warmth and peace

4. Give nitroglycerin under the tongue, or use a spray (isoket), then give a chew half a tablet of aspirin and be sure to seek medical help.

If possible, an ECG is recorded where it is necessary to determine the type of infarction - with the rise of the ST segment, without lifting this segment, also pay attention to the appearance on the film of the tooth "kyu".

Medical care for a patient with acute myocardial infarction consists of oxygen therapy, the continued use of nitrates, including injectable forms, adequate analgesia with the prescription of narcotic analgesics - morphine. If the heart attack on the prehospital stage is complicated by the development of cardiogenic shock, it is necessary to carry out a full set of anti-shock therapy: this is the replenishment of the circulating blood volume by intravenous drip injection of solutions, the appointment of hormonal drugs that contribute to the normalization of blood pressure, and the stabilization of cell membranes.

Further, at the pre-hospital stage, the question of carrying out such a treatment method as thrombolysis is solved, or questions about surgical methods of treatment are solved. In any case, the patient is transported on a stretcher to a medical institution for continuing treatment under hospital conditions.
